Facts indicating insanity observed by myself at the time of examination viz.:

He is melancholic states that he is miserable & feels tires of his life. That is unable to sleep from listening for people who are about wishing to murder him, He has been an inmate of the Infirmary of the City of London Union since January 31, 1888.

Facts communicated by others viz.:

William Evans (Attendant) states that he is low spirited, restless at night, has delusions that 2 men are waiting for him trying to murder him, says that he does not wish to live any longer.

He is rambling in his statements & has delusions of persecution. Says his enemies follow him about & persecute him. The attendants of the asylums ill-treat him thorugh the keyhole & in many other ways. Says he can hear voices talking to him though the ground.
